Billing & Insurance

Our office is credentialed with most insurance plans; please see the list below of insurance companies we currently participate with. If your insurance is not listed, please contact the office to verify that our office participates with your insurance plan prior to your upcoming appointment. A copy of your health insurance card is required at each scheduled appointment. Failure to bring your insurance card could result in having to cancel your appointment or you could be asked to be self-pay for that date of service. Our front desk staff is available to assist with any questions you may have about your insurance, however, we do not know the details of the insurance plan that you have chosen. If you have questions about benefits and covered services, please call the member services number on the back of your insurance card prior to your scheduled appointment.
